A single-page black-and-white survey map shows an elongated shoreline outline divided into numbered sections by straight boundary lines. Small survey marks and handwritten annotations appear throughout the map, with a large body-of-water label below it. A tall tabular register occupies the right side, and additional tables and handwritten notes run along the bottom edge.
