A scanned one-page order form is shown. The page includes a small photograph of a paved memorial area with three flagpoles, flags, dark rectangular monuments, water, and landscaping. Several sample engraved brick panels appear in the middle of the page, with blank lines and fields for a purchaser's name, address, phone number, email, and inscription.

Veteran's Bricks are meant to honor men and women who served in the Armed Forces of the United
States including the Army, Marines, Navy, Air Force, Coast Guard National Guard and WWII

Please print carefully the text you want engraved onto your brick order. For additional bricks use
multiple order forms. Send the form(s) to the address shown above. You will get a confirmation letter
acknowledging your order and the text you submitted for your final approval.

Non-Veterans bricks will be placed in the sidewalk leading to the memorial site. Any person, family,
business or organization can purchase one. We would like the first line to reflect some recognition of
the service of veterans but that's up to the purchaser.
